The Ryan White Patient Navigator will help patients access HIV care services and remain engaged in their treatment plans, in a professional and compassionate manner.
This position works at our Ceres East location.
Schedule is Monday – Friday, working from 8:00am – 5:00pm
Essential Duties and Responsibilities
Utilize best practices to assess the needs of Persons Living with HIV (PLWH) and connect them with internal services and/or external community resources.
Interact effectively and consistently with a diverse group of people of different cultures, ages, genders, sexual orientations, disabilities, races and religious beliefs.
Work collaboratively with GVHC’s clinical providers, clinical care team, case management, other administrative departments, as well as with any community agencies.
Assist clients with navigating the unique challenges they may have of staying in care and on treatment.
Educate and link newly diagnosed, new to care, and out of care HIV patients to GVHC’s programs, payment options, or outside services as appropriate and necessary.
Monitor patients to track progress and efficacy of their HIV care plan.
Collaborate with multi-disciplinary teams to ensure the best quality care is provided to each client.
Maintain client records including timely documentation in the electronic health record system or other database in accordance with clinical policies and procedures.
Support and engage in an active quality management program to ensure outstanding quality of care to patients and prevention intervention strategies.
Involve consumers and other local regional HIV services providers in planning and evaluating program activities through collaboration.
Support in the success of grant funded projects or programs, including, but not limited to the Ryan White HIV Services which is a Health Resources and Services Association funded program.
Additional duties and responsibilities as assigned.
Lift up to 30 pounds occasionally and push up to 50 pounds (on wheels) on rare occasions.
Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
Must have high manual dexterity.
Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
The noise level in the work environment is usually quiet.
Knowledge of advances in HIV/AIDS treatment through review of research-validated resources, curriculum, and training.
Knowledge of non-clinical programs and services available to clients.
Ability to understand and interact with people from different cultures, ages, genders, sexual orientations, disabilities, races and religious beliefs.
Excellent customer service and positive attitude.
Experience working with electronic health records, preferred.
Compassion and commitment to working with patients that have additional challenges with mental health and substance abuse, preferred.
Bilingual in English and Spanish, preferred.
Must hold a valid California Driver’s License, have reliable transportation, proof of insurance and acceptable driving history.
High school diploma or equivalent.
Bachelor Degree in Public Health, Nursing or Social Work. In lieu of degree, minimum two years of experience in HIV field with patient contact and enrolled in degree program.
Minimum one year of experience educating and counseling clients; in health promotion and medication management, preferred.
