Addiction Management Medical Practice – Cape Cod MAManagement services organization is seeking motivated Nurse Practitioners (NPs) for full-time positions. With the supervision of a physician, the Nurse Practitioner will administer complete histories and physicals on adult patients to establish and record the patient's current health status, develop a working diagnosis and treatment plan, and to provide continuing medical care. The role will provide outpatient-based care to patients with complex, possibly acute and chronic health and addiction management conditions. All requirements of a nurse practitioner in good standing.TITLE OF POSITION: Nurse PractitionerTITLE OF IMMEDIATE SUPERVISOR: Medical DoctorRISK OF EXPOSURE TO BLOODBORNE PATHOGENS: HighJOB TYPE AND SALARY: Full-time/ $115K +; Benefits: TBDESSENTIAL DUTIES AND RESPONSIBILITIES include the following, although other duties may be assigned:Works with the doctor(s) to give patients medication-aided treatment for general and addiction management medical conditions in line with a review of the medical and clinical record or after conducting a comprehensive health and perhaps other evaluation, to include appropriate lab, radiology, or other special tests required for evaluation of illness;
Monitors the client’s condition, as well as coordinating services and teaching and training activities, for patients regarding their conditions and treatment regimen;
Provides nursing care in accordance with the client’s plan of care to include comprehensive health and psychosocial evaluation, monitoring of the client’s condition(s), health promotion and prevention, coordination of services, teaching and training activities and direct nursing care;
Work with social services to ensure that the patient has a support system in place.
Must be able to work with autonomy within state law, under direction of supervising physician.
Performs and interprets, at least to the point of recognizing deviations from normal, common laboratory, radiological, and other routine diagnostic procedures used to identify pathophysiologic processes.
Manage chronic medication management patients while following all State and Federal Compliance Guidelines.
Assists with record keeping, completes appropriate patient medical chart documentation, and transcribes lab data and routine correspondence, as directed and reviewed by physician.
Refers patients to physicians as appropriate for consultation or for specialized health resources and treatment.
Ensures continuity of care by serving as a liaison between patient and other members of the multi‐disciplinary care team or other specialty areas as needed.
Participates in quality monitoring through the review of records and treatment plans for patient outcomes on a periodic basis to assure quality care.
Demonstrates a high standard of moral and ethical behavior. Demonstrates compassion and professionalism and a commitment to excellent patient care.
