Role Summary
Responsible for support of Individuals in their home and community. Responsible to for supporting the Mission while conducting work activities within the context of policies and standards established for and by .
Responsible for monitoring and role modeling health and safety for independent living, personal and social goals. To be an advocate and coach client rights while encouraging self-advocacy. Responsible for assisting individuals in recovery-based strategies and encouragement to develop community and natural supports. Responsible for coaching clients in development of skills to be independent and maintain their own home. Supports and facilitates team-based efforts with all members of the individuals team.
Essential Duties and Responsibilities
ensures individual treatment plan is implemented.
Follow prescribed directives in Psycho social rehab and personal care across settings.
provide support and advocates for client as appropriate.
assisting individuals to reach their goals/skill building for independent living.
crisis and emergency behavior management
med administration and proper documentation compliance per treatment plan in accordance to State of ME regulatory guidelines and practices
Represents the mission, Agency and client at all times
work in partnership with individual to access activities/ community resources
provides assistance in personal care and medical needs as defined in individual plan
ensure health and safety of individual
ensures nutritional/hygiene needs are met
provides transportation as defined in treatment plan and approved by Manager
Ability to comprehend and deliver services within the mental health services system and AMHI Consent decree/Mental health licensing standards
Understands and follow through with critical incident reports and mandated reporting
maintains and submits accurate and timely documentation to meet contractual/ licensing standards and policy and procedures.
teamwork – works well with others in the interest of the individuals and families we support
assist in the evaluation and reporting requirements for PQI, including contributing to the development of outcomes, indicators and setting targets.
participate in the collection and interpretation of data; and applying data to assist in improved practices for service delivery.
other duties associated with PQI to support the needs of the organization.


attends and participates in mandatory team meetings/monthly clinical supervision no less than 4 hours per month.

Job Specifications
Education and Experience
High School Diploma or GED 1 month of experience is desired.
Licensure, Registration and/or Certification
Valid State issued Drivers’ License and proof of current insurance required.
Obtain and Maintain MHRT-1which contain the components of CPR/FA, Behavior Intervention Training, MHSS and
CRMA. Attend all other trainings as required by Agency and State of Maine Licensing.
Knowledge and Skills
Ability to make well-reasoned decisions, both independently and as part of a team.
Ability to follow-through to meet individual or agency needs.
Ability to use discretion in handling confidential information, compliance with HIPAA regulations.
Coping skills, such as flexibility in dealing with issues typical to a social service setting.
Ability to meet job performance measures with minimal direct supervision and guidance.
Language and Communication Skills
Ability to communicate clearly, in English, (speaking, reading, writing, listening and presentation with individuals and
Service team members).
Computer Operation skills
Proficient personal computer skills including electronic mail, word processing and internet.
Ability to learn applicable software required for documentation.
Working Conditions
Work is carried out indoors under desirable conditions with little or no disagreeable features and outdoors exposure to changing weather conditions.
If guidelines are followed risks of exposure to hazardous conditions and blood borne pathogens is minimal. Travel Required Yes. Transportation of client in Ascentria and/or own vehicle throughout region Qualifications Behaviors Preferred Team Player: Works well as a member of a group Education Required High School or better.
