Oversees resident service plans and resident care needs in memory care community.SCOPE OF DUTIES: Duties to include, but are not limited to the following:· Follow the facility policy and procedures as written and communicate any needed changes to the manager.· Implement each resident’s service plan as written, and follow standards of care.· Reviews and responds to quarterly pharmacy reviews, assist in corrective changes.· Performs evaluations as requested by supervisor including admission and quarterly reviews. Performs evaluations of residents to include identification of wellness potential and appropriate referral.· Is aware of handling emergency procedures and is able to carry them out appropriately.· Consistently receptive to observations and concerns from staff, residents and families. Follow up on concerns and issues on the Dashboard in PCC.· Promotes teamwork and continuity of care at all times. Provides positive re-enforcement or redirection as necessary.· Implementing continual training opportunities for Caregivers and MedAides, making sure all training is documented and turned in to Administrator.· Notifies Director of all pertinent staffing issues – updates and staff morale.· Promote effective inter/intradepartmental communication. All written/oral communication needs to be tactful, respectful and convey a positive attitude and competency.· Responsible for review of all interventions and plan of prevention for all incidents.· Provide input on service plans and make changes as needed in areas where nursing services may be provided.· Ensure that all changes in condition are communicated to supervisor.· Assume the responsibility for own professional development through continuing education and workshops.· Responsible for updating service plans and scheduling/holding service plan meetings on a quarterly and as needed basis.· Developing and making changes to service plans with caregiver and medaide input.· In the event Director is unavailable may be responsible for admission duties, family/resident interventions and addressing staffing issues independently.· Incorporates choice, dignity, independent, individuality and privacy into job performance on a daily basis.· Maintains good personal appearance and adheres to dress code as outlined in the employee manual.· Perform all other duties as assigned.QUALIFICATIONS/EDUCATION: 1. Ability to read, write and following written and oral instructions in the English language.2. CPR/First Aid Certification3. Food Handlers Certification for the State of Washington4. Specialty Training in Dementia5. Must complete facility orientation, long term care worker orientation and safety training before routine interaction with residents can take place.6. Must complete on floor training as caregiver and medication tech before training for role as RCC7. Must complete and pass a criminal background check and fingerprinting in a timely manner.WORK EXPERIENCE: Must have experience in the delivery of health services. Has the potential to work in hot conditions in resident rooms. Potential risk for falling residents, heavy lifting and combative/aggressive residents. Works daily with the public, other personnel, visitors, family members and residents.RESPONSIBILITIES: Carry out all tasks and directions in a timely and efficient manner to provide the best quality service for our residents. Responsible to assure that all resident care needs are met and resident information is kept confidential.
