Candidates must be available Monday-Friday with the ability to work 8-hour shifts between the hours of 8am-8pm.Agency Summary: Casa Esperanza, Inc. is a bilingual/bicultural behavioral health treatment provider serving Boston and the Merrimack Valley. Our mission is to empower individuals and families to recover from addiction, trauma, mental illness and other chronic medical conditions; overcome homelessness; and achieve health and wellness through comprehensive, integrated care.Job Summary Statement: The role of the Health and Recovery Specialist is to provide case management and non-clinical recovery support services intended to aid individuals in establishing and maintaining individual recovery from addiction and mental illness and maintain health and wellness.S/he will provide information about medical, psychosocial, and recovery support services to help patients improve their quality of life and conduct community outreach to identify resources in the community that support patient needs. This individual will also facilitate Health & Wellness and Recovery Support Groups that address relapse prevention, HIV risk reduction strategies, health literacy, and mental health.As part of the Casa Esperanza, Inc. team you will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach, to ensure the coordination of all client services, both across Casa programs as well as with external providers. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; details necessary follow up and assignments; and ensure-wrap around support services are in place to maximize treatment effectiveness and clinical outcomes.Essential Functions: Outreach & EngagementAttend and participate in community networking meetings and outreach events to develop relationships with community agencies who may need support connecting PLWH/A to behavioral health services
Coordinate and conduct admissions and intake for clients to the Familias Unidas Outpatient Program, as appropriate
Provide outreach and aftercare follow-up to maintain engagement and re-engage clients who drop out of care
Help patients to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and patient’s part in creating them
Collects client data and conduct follow up surveys to ensure long-term client engagement
Assists in coordinating pro-social, recreational and cultural activities for patients

On-Site Service DeliveryFacilitate weekly Health and Wellness groups and other HIV and/or recovery support groups, as needed to provide patients with ongoing education regarding substance abuse, mental health, HIV/AIDS, Hepatitis, tobacco, nutrition and exercise and other health and wellness information
Conduct initial assessment for each client and generate individualized service plans that reflect each client’s unique strengths, needs, abilities and preferences, as appropriate, and aligns with their Recovery Goals
Conduct periodic re-assessments of the ISP including review of utilization of services and client-specific advocacy
Connect population to HIV and Recovery support services at Casa Esperanza and at other agencies across Greater Boston.
Assist clients in choosing a Primary Care Provider, making their first appointment, obtaining health insurance, and how to discuss their substance use with PCP
Refer clients to appropriate outside services such as: medical, behavioral health, housing, financial, legal, education, training, employment, life skills, peer support and other services
Assist patients in navigating recovery support systems and accessing resources including medical, health and wellness, housing, employment, entitlements, and other professional and non-professional services;
Coordinate client transportation as needed, including medical and social support appointments.
Provides interpretation for clients, as needed

AdministrativeDocument all client encounters and other data as required and maintain up-to-date client records
Work closely with multidisciplinary team to ensure that client’s treatment and service plans are implemented and client needs are being met
Attend bi-weekly supervision
Assist in all reporting as required by Program funding sources
Attend all relevant case conferences and staff meetings including MDT
Other duties as assigned

Requirements: Bilingual Spanish/English required
Experience working with individuals living with HIV, addiction, mental illness, trauma and violence
Minimum 2 years demonstrated personal recovery experience, if applicable.
Knowledge of care coordination and community resources
Knowledge of co-occurring disorders
Understanding of harm reduction techniques
Strong interpersonal skills;
Crisis management Skills
Strong verbal and written communication skills in English
Ability to work as a team member and independently
Knowledge of Ryan White Standards of Care, preferred
Computer skills, minimum Microsoft Word
Valid Massachusetts Driver’s License
Ability to meet the physical and training requirements of the position

Education/Training Required: Minimum High School Diploma required; bachelor’s degree in related field preferred.
Minimum of 2 years’ experience providing Case Management or Recovery Support services
Certified Alcohol and Drug Counselor/Certified Alcohol Counselor required or willing to obtain within one year of hire
CPR certified
