Role: Ensures primary coordination of release of health record information including tracking of purged, stored records according to HIPAA rules and regulations. Assists with scanning and indexing of files and other special projects related to medical records.Essential Duties and Responsibilities: Perform all job functions according to the mission and values of CCHC.
Ensure the quality of medical/health records by verifying their completeness, accuracy, and proper entry into computer systems.
Use computer applications to assemble and analyze patient data for the purpose of improving patient care or controlling costs.
Receive and process incoming and outgoing release of information forms and document accordingly on the spreadsheet.
Fill requests over the telephone from hospitals needing records for continued care – without a release; take out any mental health, substance abuse, HIV/AIDS or ask for an ROI.
If fees are required, make invoices and document on spreadsheet. When payments are received, document on spreadsheet for outgoing. Follow up on any payments we haven’t received yet.
Scan in and document on spreadsheet medical records that are received per our request. Follow up on records not received on a weekly basis.
Assist patients over the phone with their medical records questions or assisting them in completing the release of information forms. Mail out releases with instructions when requested.
Change patient demographic information on the registration screen when necessary.
Mark patient as deceased in registration when appropriate confirmation has been received. Put pop up alert in patient’s chart when patient has transferred out.
Develops and provides training for staff on identified client record/release issues.
Assists with scanning and indexing of medical record files by collecting paper documents of medical/dental record to be scanned on a daily basis.
Confirms patient information with a minimum of two identifiers prior to scanning into record.
Scans document into computer and transfers to appropriate location within the chart.
Confirms that provider, date, and patient information is accurate prior to signing.
Routes to appropriate provider for signature if needed.
Corrects documents that were imported incorrectly.
Assists in quality assurance processes by double-checking the work of other medical records staff at a rate of, on average, 10% of all documents scanned per day.
Destroys records according to clinic policy and state regulations.
Assists with other insurance, billing and coding projects as needed.

Education and Experience: Minimum of high school diploma or GED required.
Registered Health Information Technician (RHIT) certification and a minimum of one-year work experience with electronic medical records required.
