is searching for candidates interested in a dynamic social work opportunity in health care. Case Management is a department that supports all areas of the hospital, including Acute Care, Acute Mental Health, addiction treatment at Rainforest Recovery, and the Emergency Department. Case Management is comprised of nurses and social workers that work together to provide coordinated care for our patients. The current recruitment is for out-patient services at Rainforest Recovery, however, initial case management orientation will be in acute care. Social Work Case Managers generally have a primary location, but are expected to cross train to other areas and may be required to work in other units, based on organizational need. This role is in a fast paced environment and is responsive to multiple teams and partners. Beyond the direct patient care, the case managers are involved with identifying programs, services, and projects to support patient and community needs. They participate with community coalitions to collaborate and support coordination of care and improvement in services. Discharge coordination: Synthesizing information obtained from assessments and the multidisciplinary treatment team to develop a comprehensive, safe discharge plan in a timely and efficient manner that supports the patient and treatment team goals.
Participation with the multidisciplinary treatment team with a strong capacity for providing recommendations to the treatment team: Attending rounds and bringing information from assessments forward and making recommendations based on the integration of information from SW assessments and other treatment team member assessments. Scheduling and participating in patient treatment team meetings.
Initial and ongoing assessment: Assess patient for discharge planning needs and for high-risk factors indicating special discharge planning needs. Evaluation of continuity of care needs. Assessment of resources.
Referral: Identifying and implementing referrals in a timely manner to be able to transition the patient to the appropriate level of care as soon as they are stable to make that transition while avoiding gaps in service or payment.
Problem identification and health and behavioral health related goal setting: Clarifying the patient needs, wishes, and preferences and establishing agreed upon goals of care for the next level. Often includes treatment team meetings.
Advocacy: Serve as a resource when needed to speak to patient needs with the treatment team and community partners and providers.

Minimum Qualifications for the Job:
Social Work Case Manager I ($31.80$37.41 / hour)
Education: A minimum of 3 years of postsecondary education within a social work program. Graduate of an accredited social work program (BSW or MSW) within one year of hire.
Experience: Experience in hospital case management or healthcare preferred.
Social Work Case Manager II ($35.23$41.45 / hour)
Education: Graduate of an accredited social work program (BSW/MSW). Completes annual educational requirements.
Experience: Advancement from the Social Work Case Manager I (at least one year) or a minimum of one year recent experience in a comparable hospital setting.
Social Work Case Manager III ($37.37$43.97 / hour)
Education: Graduation from an accredited program with a Master's Degree in Social Work; or a Bachelor's Degree in Social Work and a Master's Degree in a related field.
Experience: Minimum of 1 year working as a social worker in the area of health care, behavioral health, or substance abuse treatment.
License: Preferred ability to obtain LCSW.
