With our signature wrap-around approach, which includes expert case management and mental health treatment, we have raised the bar when it comes to services offered to the community.We are looking to make an immediate hire for a Transition Coordinator.The Transition Coordinator is a member of an interdisciplinary team that consults on referrals of custodial children and youth to Superior Caregivers’ services. The Coordinator is the primary liaison to the Department of Children’s Services (DCS) regional and central office staff regarding all new referrals and necessary follow-up on in-progress referrals. The Coordinator communicates the team’s decision if Superior Caregivers can safely serve the child/youth. The Coordinator works with Superior Caregivers interdisciplinary team to prepare information for monthly Utilization Reviews (UR). The Coordinator represents Superior Caregivers on DCS regional utilization review calls and communications the child/youth’s status and progress in services. The Coordinator also advocates for the child/youth to obtain additional services and supports based on the child/youth’s needs.BenefitsWork from home (occasional meeting in Dickson, TN office might be required)

Essential Functions and ResponsibilitiesAvailable to DCS staff during and after business hours to receive, review, and respond to new referrals.
Review all incoming referrals and collaborates with the interdisciplinary team regarding the child/youth’s needs.
Conducts meet and great (in-person, Facetime, Skype or Zoom) with child/youth and his/her DCS case manager or therapist to build rapport, introduce services, and form initial impression of child/youth’s service needs.
Contact potential Superior Caregivers foster families to present the child/youth and obtain a decision on their ability to serve the child/youth.
Communicates team decision to DCS regarding availability of a Superior Caregivers foster home to meet the child/youth’s need.
Communicate with DCS staff, whether region and/or central office staff, about the status of pending referrals.
Track referrals and waitlist, as appropriate.
Attend Child Family Team Meetings (CFTMs), as appropriate.
Participate in weekly DCS calls and attend required meetings at the HUB.
Work with the foster home support specialists to maintain knowledge of approved homes.
Work with foster parent trainer to know about any pending homes in process.
Work with Director of Juvenile Residential Services to know availability of supportive living homes.
Works with program coordinator to identify missing documents/forms and works to obtain them.
Participate in internal utilization reviews (UR) to prepare information for DCS lead URs.
Review and enter Serious Incident Reports (SIRs) into TFACTS.
Attend collaboration meetings with community partners, as needed.
Comply with the federal Confidentiality Law and HIPAA regulations and guidelines to protect the rights of the client.
Comply with all Superior Caregivers and related DCS Policies and Procedures
Effectively communicate with Superior Caregivers’ Foster Home Team.
Display professional and effective verbal and written communication skills.

QualificationsBachelor’s Degree in Social Work, Psychology or equivalent in closely related field
Experience working with Department of Children’s Services (DCS) and/or child welfare system, preferred.
Must possess professional and expert knowledge of communications, as well as building and maintaining relationships.
Strong organizational abilities including filing, attention to detail, form completion, professional writing, etc.
Must be able to and willing to undergo in-depth background and reference checks

Job Type: Full-timePay: $20.00 - $23.00 per hourBenefits:Flexible schedule
